What if you don’t want to give us the requested information, or if you change your mind
If you do not provide this information, or if you retract your consent at a later date, it will mean we are unable to provide this service and you will need to be withdrawn from the program.
Other uses of your de-identified personal information
Your de-identified information may also be used for statistical or research purposes. This means that any personal information that could reasonably identify you will be removed or changed before it is used for this purpose.
